NO there are no such steps. You are getting confused with the stages of a claim. If a DRO is involved it is an appeal. Two different animals Claim&Appeal.

YES, if they opened it as a claim, but the DRO should not be involved, unless the DRO found the CUE but that means the DRO found it during the appeal process.
